Connecticut does not provide forms to be completed when there has been an accident. All information must be given to and reported by the State Police.

If you have been in an accident in CT and the State Police have taken your report, be sure to get a copy and send it to the Risk Manager.

If the State Police do not respond to your accident, please fill-out a MA Crash Report and send it to the Risk Manager [This report is for reporting the details of the accident only, do not send to the Registry of Motor Vehicles].
