How to Become a Credentialed Driver at HC

In order to comply with the insurer's requirements that we credential drivers, we must collect from each person who is to be authorized to drive a College-owned, leased or rental vehicle (including motorized equipment and/or unregistered equipment):

  1. A signed, completed Annual Driver License and Record Update form.
  2. A photocopy of each person's driver's license.
  3. A signed agreement to the list of College "rules for vehicle use" on the back of the Annual Driver License and Record Update form.


The College provides the insurer with a comprehensive list of these drivers, including their names, dates of birth, driver's license numbers and state of issuance. Our insurer will use this list to check the status and history of all authorized, credentialed drivers.


Individuals who are affected by this requirement are those who:

  • Must drive as an essential part of their duties and responsibilities.
  • Have a College vehicle specifically assigned to them.
  • Drive a College owned, leased, rented or personal vehicle on college business, including motorized equipment and/or unregistered equipment.
  • Carry passengers in College-owned, rented or in personal vehicles as part of College business.
  • Regularly or frequently drive their personal vehicle on College business.


Persons who drive only their personal vehicle on College business occasionally should also seriously consider submitting information to be a credentialed driver. Five Colleges carries insurance up to million. A serious "at-fault" accident with a loaded passenger van could easily exceed personal insurance and put personal and family assets at risk. Being credentialed will enable the driver to be covered by the College's excess insurance while driving a personal vehicle on College business.


Department Chairs or Heads are responsible to ensure that all authorized drivers complete the Annual Driver License and Record Update form and provide a photocopy of their driver's license, for collecting the documentation and forwarding it to Nancy Karella in the Treasurer's Office. The forms, photocopies and the above mentioned list must be completed and submitted electronically to Fred C. Church Insurance by September 30th of each year.


Drivers not previously credentialed or new employees need to credential as soon as possible before they may drive a College or rented vehicle.
