Exchanging proof of insurance is a common step in the contracting process. A Certificate of Insurance is a document obtainable by request from your insurance company as proof to a third party that your institution is insured.

Exchanging proof of insurance is a common step in the contracting process. A Certificate of Insurance is a document obtainable by request from your insurance company as proof to a third party that your institution is insured. This document lists all of the information required to file a claim in the event of an accident or injury: the name of the insured, the policy numbers, the type of coverage, and the limits on coverage.

Submit Request for Certificate of Insurance

The Office of Risk Management manages the commercial insurance portfolio for the four private colleges within the Five College Consortium: Amherst, Hampshire, Mount Holyoke and Smith. Upon filling out the form below, your request for a certificate of insurance will be sent to the appropriate contact, and a certificate will be issued to the parties specified. Within five business days of placing your request, a Certificate of Insurance will be generated by our insurer. Property Proof of Insurance Request Details

Our insurer, FM Global has the following additional questions for those who require proof of Property Insurance.
Which of the following do you need?
Certifies that in the event of a loss, the college will include the third party interest in any loss payment. The college is responsible for any additional rights granted by the certificate to the third party.
Confirms that insurance is in force for an insured location. Document does not grant additional policy rights to a third party interest.
If the event for which this proof of insurance is being requested will affect a specific building or cluster of buildings, specify which buildings here.
