What is a Certificate of Insurance?

Exchanging proof of insurance is a common step in the contracting process. A Certificate of Insurance is a document obtainable by request from your insurance company as proof to a third party that your institution is insured. This document lists all of the information required to file a claim in the event of an accident or injury: the name of the insured, the policy numbers, the type of coverage, and the limits on coverage.

Submit Request for Certificate of Insurance

The Office of Compliance & Risk Management manages the commercial insurance portfolio for the four private colleges within the Five College Consortium: Amherst, Hampshire, Mount Holyoke and Smith. Upon filling out the form below, your request for a certificate of insurance will be sent to the appropriate contact, and a certificate will be issued to the parties specified.
